Survey & Assessment: Add fields to survey template library

To help with managing content and making templates accessible to certain teams, add 2 data fields to a template: Applicable practice team(s) and applicable integrated solution(s). This can allow us to filter and show relevant content when creating a survey via template instead of a large menu.

Some teams also dont want to expose their survey templates to other practices, e.g., Deals so they wont adopt.
