When documenting a decision, there are several things that a PM would want to document that are missing as pre-populated items in Concourse. For example - what forum was the decision made (or will be made) in? What was the actual decision (ex: Accept, Deny, Fix & Return, etc). What are the next steps, if any? You don't need a 'due date,' but you'd need a "decision date" or "decision required by" date. Just some ideas. I think this needs to be rethought through.
